Once packet is completed and recieved,
enrollment will be completed upon receipt of $65.
What am I paying for?
$30 of your payment is for enrollment.
$35 of you payment is for your class booklet.
Once payment is confirmed, you will make an appointment with one of our staff members to pick up your booklet.

Fill out Enrollment Packet
All applicable information must be filled in and truthful.
4.
Download the completed packet
This button can be found in the upper right-hand corner of the screen, once you select it, select the "Edited" option.
5.
Attach to your email and send!
Send completed emrollment packets to mekia@abalancedapproach.org
Once enrollment packet and payment is recieved, a member of our staff will be in touch to book an appointment to pick up your class booklet.
